You win the spade lead in your hand and play the Queen of Clubs. This loses to the King, and you get back the Queen of Hearts. You play low and jammer plays the Jack of Hearts. This sequence of plays is consistent no matter who holds the Ace of Hearts. Remember that the correct lead with a suit headed by the Queen-Jack with or without the Ace is the Queen.
Out of context, you should put up the King. The probability of the Ace on your right exceeds that of Ace and two small on your left.
But step back a bit. Can you make this contract if the King of Diamonds is on your left? If you consider carefully, you will conclude no. With that in mind, would jammer pass originally with the Ace-Queen-Jack of Hearts, King of Clubs and King of Diamonds? No, he would not. So, your only hope is to play low again, hoping to block hearts.
Contract: 3NT
